WHO CAN JOIN? Primary care providers are ideally positioned to inform system level change in health care delivery. If you are a primary care provider in North York, join the NYTHP PCN to be a part of the co-design of the OHT, connect with other providers in the community and learn about new OHT programs to support your patients and your practice right in the North York community. By joining the NYTHP PCN I understand the following membership benefits and rights:
- PCN membership is voluntary for any primary care providers with patients who use services in the NYTHP OHT region;
- Membership in the NYTHP PCN will not be legally binding and primary care providers may withdraw at any time;
- PCN members shall elect the Primary Care Advisory Council (PCAC) to represent the broader PCN membership;
- Members shall receive communication updates regarding relevant initiatives and programs in the NYTHP OHT
